Community Loving Care Hospice, L.L.C. staff and volunteers unite with the community to accomplish a single goal: To provide the patients of our Hospice Program and their families the highest quality of care possible so they may live each day to the fullest.
Community Loving Care Hospice, L.L.C. provides care, employs staff and offers volunteer opportunities to all persons equally, regardless of race, color, religion, creed, gender, national origin, disability, age, diagnosis or sexual orientation.
When faced with a terminal illness, we all hope to receive compassionate care in a comfortable environment. For some of us, that will be in our own home. For some, that care will be in the home of a family member or of a friend. Others will receive end-of-life care in a long-term care facility. Whatever the setting, CLC Hospice will provide caring and compassionate Hospice services to our patients and their families.
- Skilled nursing care specializing in Pain Management, Symptom Control, Education and Emotional Support.
- Home Health Aides and Certified Nurse's Aides to provide total personal care.
- Social Workers to provide grief counseling and to assist in accessing community resources.
- Pastoral care for spiritual services.
- Physical, occupational and speech therapies for palliative needs.
- Arrangements for medical equipment, supplies and medications related to the terminal illness.
- General inpatient hospitalization for acute conditions unable to be managed in the home.
- Hospice care for patients in nursing homes and assisted living facilities.
- Bereavement support for family for at least one year following the patient's death.
- Grief counseling for adults and children who are ill or who's loved one is ill.
- Help is always available 24 hours a day, seven days a week.
- Grief and Loss support groups are available.
- For Patients who desire, CLC Hospice will write a biography for the family to have as a keepsake.
Respite Care
When the patient resides at home and the family or a friend is providing continuous care, the need for a break may arise. Respite care is available to meet the needs of the patient and the family or friend. Respite care can be for a couple of hours while a Hospice volunteer or staff member stays with the patient. Respite care may also be when the patient is temporarily placed in a facility so that the family or friend can get much needed rest from care giving.
Pastoral and Bereavement Services
Everyone's image and approach to a spiritual life is a very personal issue and Community Loving Care Hospice respects and supports our patients' wishes in this regard. Our team works with your religious representative, if requested, to ensure that the religious resources of your tradition are available during the illness. We provide spiritual counseling, reception of the sacraments, spiritual reading materials, or someone to talk with. For those who desire, we will leave spiritual support to the family and the patient without intruding in their personal decisions.
Our hospice aspires to do whatever fulfills the wishes of the patient and family regarding this personal matter. We will, of course, be there during the period of bereavement. Each of us grieves in our own way and on our own schedule. Working through the grief process can also be a time of personal growth with appropriate resources for support. Community Loving Care Hospice will continue to be that resource as long as the need prevails.
